Opening Hearts accepts referrals from approved agencies, county partners, and authorized professionals.

Referrals may be initiated by phone or email. Once a referral is received, our team reviews the youth’s needs, background information, and placement goals to ensure an appropriate and supportive fit.

We work collaboratively with referring agencies, caseworkers, and families to coordinate intake, documentation, and transition planning. Our priority is providing a safe, stable, and trauma-informed environment that supports each youth’s long-term growth and well-being.
